The following is a checklist guide for Weightloss Unlimited telemedicine practices.
Enrollment and Consent for Medical Services
- Potential patient requests the HCG Diet Protocol by signing up at Weightlossunlimited.com
- The potential patient agrees to the Privacy Policy, HIPAA Disclaimer, and Terms and Conditionsat weightlossunlimited.com.
- The potential client fills out the Medical History forms.
Medical records and initial consultation
- A practitioner of Weightloss Unlimited looks over the potential patients medical records as per: GEORGIA GUIDELINES
- The practitioner then consults as necessary with the potential patient ( and documents the evaluation, adequate to diagnose the client as overweight or obese, based on the BMI chart.
- A Weightloss Unlimited specialist calls the patient and confirms that the patient understands the services and protocols, and has all the necessary materials, including workbooks, injection directions, and any other necessary materials. The specialist makes notes to the patients file as necessary for the practitioner to review.
- After review, the practitioner issues a prescription based on the Medical history, telemedicine evaluation, BMI charts, available blood work, a physical doctor evaluation or recommendation, and speaking with a weight loss specialist to evaluate weight loss concerns and protocols.
Weightloss Unlimited and their medical staff understand and agree that the methods for issuing a prescription to patients based on these items is an acceptable standard of care, solely for our non-invasive, elective services.
Welcome Orientation and Instructions - A Weightloss Unlimited representative calls the patient to welcome him/her as a patient and confirms that the client has received all the necessary training and materials to perform the diet protocol.
- After the above takes place, the patient is then charged for the consultation and fees for the HCG Diet Protocol.
Follow-up Consultations
- At the two week point, the client is called medical assistant to ensure the client has received all of their items from the pharmacy, and to address any concerns.
- At the three week point, the client is called by a weight loss specialist to go over the diet and any questions, concerns, and information that Advanced HCG Diet Center feels is necessary for the proper care of the patient.
